How to complete the ABS-CBN Application Form

There are 5 sections in the application form:

1) Top (To be filled out by your dealer)

2) Subscriber (To be filled out by you)

3) Representative (To be filled out by your dealer)

4) ) Warehouse (To be filled out by ABS-CBN)

5) Acknowledgement (To be filled out by you)

Ad 1) Information when your application has been received and the identification assigned by your dealer.

Ad 2) Your personal data. Please fill out as accurate and legible as possible. All information is confidential and will not be used for any other purpose.

Item: Type of subscriber: Mark: Dealer

Ad 3)

Items Mode of Payment: Only full payment is accepted. When you need to finance the amount, please arrange with your bank.

Ad 4)

The IRD No. Identifies your receiver. Both the smart card number and the IRD number have to agree, otherwise your receiver will not work. This information will be transmitted as soon as the application form has been received by ABS-CBN via the satellite. This will activate your unit. We will give you the estimated time when the activation will be send, Please have your receiver running at that time.

Ad 5)

Acknowledgement: This is your final signature which will bind you to the contract. Without your signature the form will not be processed.

5) Acknowledgement (Muss von Ihnen ausgefüllt werden))

Ad 1) Die Information, wann Ihr Antrag beim Händler angekommen ist, und die Registrierungsnummer, die Ihnen vom Händler zugewiesen wird.

Ad 2) Ihre persönlichen Daten. Bitte füllen Sie diesen Abschnitt so vollständig und leserlich aus wie möglich. Alle diese Daten werden im Sinne des Datenschutzgesetzes nur für diesen Antrag verwendet

Punkt: Type of housing: Wenn Sie in einem Mehrfamilienhaus wohnen, stellen Sie bitte sicher, dass die notwendige Satellitenanlage auch von Ihnen installiert werden kann. .

Item: Form of Payment: Debit to account. Kreuzen Sie das an, wenn Sie das Geld überweisen wollen. Bitte ankreuzen: <Bank Draft> wenn Sie das Bankeinzugsverfahren bevorzugen. Kreditkarten können wir noch nicht akzeptieren. Und vergessen Sie nicht Ihre BLZ (Bankleitzahl), wenn Sie das Einzugsverfahren wählen

Ad 4)

Die IRD No. Identifiziert Ihren Receiver. Beides, Ihr Reciever und Ihre Smartkarte müssen übereinstimmen, sonst funktionieren die nicht. Die Aktivierung Ihrer Karte wird von ABS-CBMN per Satellit übertragen, sobald Ihr Antrag vorliegt und Sie bezahlt haben. Wir werden Sie informieren, zur welcher Zeit ungefähr das Signal gesendet wird. Bitte schalten Sie den Receiver dann ein, damit er das Signal verarbeiten kann

Acknowledgement: Das ist Ihre verbindliche Unterschrift. Ohne diese Unterschrift kann Ihr Antrag nicht bearbeitet werden. .
